Referrals

If you need to make a referral for a patient or student you work with, you’ve come to the right place. Oaklawn’s Access Center exists to help anyone in our community connect with the services they need – whether that’s here or somewhere else.
To make a referral to our Access Center, call:

If you make a referral, please inform the individual or their parent/guardian that you’ve made a referral. We will work with them to determine if Oaklawn is the best fit, or we may refer them to a private clinician in the community.
We speak English and Spanish and have access to interpreters for other languages. Regular hours are from 8 a.m. to 5 p.m. Monday through Friday, but the center takes emergency requests after hours.
If you believe the person you’re calling about is a danger to themselves or others, call 911 or take them to your local emergency room if it is safe to do so.
St. Joseph County
If you work at a school in St. Joseph County, you can make an online referral too.
Please note, we are experiencing a high volume of referrals and do have a wait for services. If this is an urgent need such as suicide risk, please call Kim Neff who will help evaluate the situation. Kim can be reached at (574) 283-1234, Ext. 4219.
