Make a Mental Health Referral in Goshen, Elkhart, Mishawaka or South Bend Indiana
Refer to Outpatient Services
The best way to refer someone to Oaklawn is to share information about our Open Access clinics, where people can walk in to begin services – no appointment needed.
If you choose to send us referral information ahead of time, we will hold it until the individual comes in. If they decide to start services with us, that information can help inform our assessment and care.
Please note that we do not contact individuals to schedule them to come in for services. For that reason, it’s important to share our Open Access information directly with the person you’re referring.
Before making a referral, we recommend reviewing our services to be sure the service you’re seeking is one we offer: See Mental Health Services and Addiction Services. If you have questions or would like help determining the best next step, email our Access Center team.
Request or Download Printed Materials
